As a CCH patient, you have access to a care team of providers including primary care providers, registered nurses, licensed social workers, community health workers and peer support specialists, all of whom are actively engaged in your care.

To reach a provider after hours, call 313-831-3160 and follow the prompts to speak to the provider on-call.

Adult Medical Care

At Central City Health, we believe that everyone, regardless of health status or personal circumstances, deserves high-quality, compassionate health care services. As a CCH patient, you have access to a care team of providers including primary care providers, registered nurses, licensed social workers, community health workers and peer support specialists — all of whom are actively engaged in your care.

Our team of providers specializes in whole health wellness to ensure that all patients, regardless of age, race, sex, gender or health status, receive the individualized care they need. Our primary care team provides all routine medical care including:

  • Diagnosis and Treatment of Acute and Chronic Health Conditions
  • Routine Preventative Care
  • On-site Laboratory Tests
  • Counseling regarding Healthy Behaviors
  • Diagnostic Imaging and Specialty Care Coordination through Referrals
  • After-Hours Advice for Urgent Medical Needs
  • Vaccines/Immunizations
  • Minor procedures such as joint and soft-tissue injections

In addition to physical health, your care team will work with you to address emotional and lifestyle factors that contribute to your overall wellness including, living situation, stress factors, diet, personal relationships and high-risk behaviors. We work with you to address your concerns and develop a treatment plan that attends to your specific needs.

CCH accepts walk-ins and same-day appointments, and we accept all insurances, Medicaid, as well as people without insurance.

Pediatric Care

Central City Health recognizes that children’s unique and changing needs depend on a variety of support systems. Key elements of successful support systems attend to the needs of the child and the family in the context of their emotional, physical and social health. CCH believes that team-based care involving medical providers and community partners is a crucial and necessary component of providing high-quality care to children and their families. Our pediatric providers deliver services for children including:

  • Sick and Well Child Visits
  • Recommended Vaccinations
  • On-site Laboratory Tests
  • Counseling regarding Healthy Behaviors
  • Diagnostic Imaging and Specialty Care Coordination through Referrals
  • After-Hours Advice for Urgent Medical Needs
  • School physicals/Sports physicals
  • Lead testing

Provider staff performs evaluations and age-appropriate screenings to aid in mental, physical and emotional growth and development. Staff advises patients, parents or guardians concerning, diet, activity, hygiene and disease prevention and recommends interventions to modify behaviors associated with health risks. Women’s Health Care

The role of a primary care provider in women’s health is vital. Primary care providers are often the first point of call for health care concerns and needs. This often opens a pathway to implement preventive health measures, including:

  • Pregnancy testing
  • Cervical cancer screening
  • Breast health
  • Resources

Your Care Team Will Work Hard To:

  • Listen to your health concerns
  • Answer your questions
  • Provide you with a team of professionals that will focus on all aspects of your health and wellbeing
  • Provide easy access to services by offering extended office hours and after hours consultation by phone
  • Communicate test and treatment results promptly
  • Provide reminders for follow up and preventative care
  • Coordinate your care with other health care providers on site and through community referrals
  • Provide you with the treatment and guidance you need to care for yourself. 

We Ask That You:

  • Provide a complete medical history, including information about care received by outside providers
  • Get to know your health care team and help them get to know you. Don’t be afraid to ask questions!
  • Work with your care team to set your own treatment goals to improve your health
  • Make your best effort to attend each appointment or call us at least 24 hours before your visit if you need to reschedule
  • Share your medical history by asking any other providers that you see/have seen, to send their reports to your PCP
  • Bring any medications, vitamins or supplements you are taking to your appointments
  • Bring a list of questions for your care team so that we can coordinate your care together!
