Community Re-Entry Division
Central City Health’s Community Re-Entry and Returning Citizens services are devoted to individuals returning to the community from the criminal justice system, or who may benefit from mental health services instead of serving time in jail. Without assistance, too many individuals released from incarceration repeat the negative cycle. CCH’s Community Re-Entry and Returning Citizens services are designed to give those with a history of incarceration the job and life skills needed to be successful in the community. Individuals being released from the jail and/or prison system need comprehensive and considerate health care services and community support. However, without appropriate resources this population often goes unserved.
Candidates for CCH’s Community Re-Entry and Returning Citizens programs may be walk-ins with a history of incarceration, Mental Health Court and specialty court cases, currently on probation and parole. Participants receive intensive services including case management, psychiatric services, peer support, health care coordination, and employment assistance.
COMMUNITY RE-ENTRY AND JAIL DIVERSION

Mental Health Court
Mental Health Court is a collaboration between CCH, Wayne County’s Third Circuit Court, the Detroit Wayne Community Mental Health Authority, and the Wayne County Department Health. Individuals who choose to participate in the Mental Health Court are diverted from lengthy jail sentences. Eligible participants are those who have committed non-violent felony crimes believed to be directly related to symptoms of a mental illness. CCH works closely with court personnel to assure that individuals are provided with the services and supports needed to become active participants in the community.

Court Ordered Services
Assisted Outpatient Treatment (AOT) is a court-ordered program designed for individuals with serious mental illness who need ongoing support but may not voluntarily seek or comply with treatment. It provides necessary mental health care to prevent hospitalization or incarceration. Central City Health (CCH) is an identified community mental health agency (CMH) that has consistently worked with Wayne County Courts as well as the Michigan Department of Corrections to provide services to members with an AOT order. If the court has sentenced an individual for a crime and has deemed the individual not guilty by reason of insanity (NGRI), then they will be court ordered to treatment. Members with NGRI status will have an AOT order and may also receive more intense services i.e. Assertive Community Treatment (ACT). Here at CCH, we work diligently to provide quality service to members who have been directed to the agency from the courts. We are committed to empowering individuals with behavioral health challenges to lead fulfilling lives.
Your Care Team Will Work Hard To:
- Listen to your health concerns
- Answer your questions
- Provide you with a team of professionals that will focus on all aspects of your health and wellbeing
- Provide easy access to services by offering extended office hours and after hours consultation by phone
- Communicate test and treatment results promptly
- Provide reminders for follow up and preventative care
- Coordinate your care with other health care providers on site and through community referrals
- Provide you with the treatment and guidance you need to care for yourself.
We Ask That You:
- Provide a complete medical history, including information about care received by outside providers
- Get to know your health care team and help them get to know you. Don’t be afraid to ask questions!
- Work with your care team to set your own treatment goals to improve your health
- Make your best effort to attend each appointment or call us at least 24 hours before your visit if you need to reschedule
- Share your medical history by asking any other providers that you see/have seen, to send their reports to your PCP
- Bring any medications, vitamins or supplements you are taking to your appointments
- Bring a list of questions for your care team so that we can coordinate your care together!
