WHAT WE DO

Target Population and Recognitions

Central City Health has been serving the residents of Detroit and Wayne County since 1972, providing whole health wellness services to all people, regardless of their situation. CCH has a strong bond to the community and does not discriminate based on race, color, religion, gender, gender expression, age, national origin, disability, marital status, sexual orientation, military status, or inability to pay for services.

CCH is recognized as a Level III Patient-Centered Medical Home, the highest level of recognition from the National Committee for Quality Assurance (NCQA). The PCMH accreditation recognizes practices successful in delivering evidence based, team-based, integrated health care with the intent of obtaining maximal health outcomes for patients.

CCH is also a Health Services and Resources Administration (HRSA) funded Federally-Qualified Health Center, staffed by licensed health care professionals who provide quality care to all people regardless of insurance status or income level. CCH believes in providing comprehensive, compassionate care with dignity and respect.

At CCH, patients have access to a care team of providers that focus on all aspects of their health and overall well-being, including emotional, family and social concerns. Available services include:

• Supportive Housing
• Community Re-Entry Services
• Supported Employment
• Insurance and Community Resource Assistance
• Peer Support

In 2018, more than 4,400 consumers took advantage of CCH’s menu of available services.

Our History

CCH has a long history of providing evidence-based medical and behavioral health treatment and services, in tandem with creating housing, employment and community re-entry opportunities for Wayne County residents. Central City Health (CCH) was founded in the late 1960s as part of the “Model City Neighborhood Program” and incorporated in 1971 as Detroit Central City, a 501(c)(3) private non-profit agency.

2014 Detroit Central City Health Center opened in a temporary storefront at 3427 Woodward Avenue
2013 Board mission statement changed to include integrated health care; DCC awarded new access point from HRSA to open a Federally Qualified Health Center
